Identity Beyond Diagnosis Program Manual
Identity Beyond Diagnosis is a program designed to support individuals, affected by cancer in developing and maintaining their occupational identity beyond their diagnosis. Through six educational modules within this manual, the program explores key topics related to occupational identity and provides practical, action-based strategies to help participants continue engaging in meaningful daily activities, roles, and routines. The goal of the program is to empower adolescents to recognize their strengths, adapt to life changes, and foster participation in everyday life despite the challenges associated with cancer. Please note that this resource was designed for adolescents, ages 10-14, however it is available to anyone who wishes to use it and can be meaningful to individuals of all ages!

The purpose of this Community Occupational Therapy Resource Guide is to provide an evidence-informed resource for future occupational therapy doctoral students completing community-based experiences and for community organizations interested in partnering with occupational therapy practitioners or students. This guide highlights the value of community-engaged learning, introduces opportunities for occupational therapy practice within community settings, and serves as a foundation for developing meaningful, collaborative partnerships that promote health, participation, and well-being across diverse populations.

Client/Caregiver Resource Contents
These client and caregiver resources are designed to support individuals and families in managing the complex schedules often associated with healthcare needs. The resources include a variety of organizational tools that provide structured systems for planning, tracking, and monitoring health-related needs, routines, and concerns. These supports are intended to reduce caregiver burden, promote consistency in care, improve adherence to medical recommendations, and enhance confidence in managing daily health-related responsibilities.
Medical Contact List
· A contact list sheet to store contacts for your healthcare team. May include name of professional, their specialty (doctor, nurse, therapy, etc.), the name of the office/facility and address that they are located at, and the phone number and/or email to reach them.
Daily Schedule
· A daily planning sheet that includes a time schedule by the half hour to plan out your day and sections to include your priorities, reminders, and a to do list for the day.
Medication Tracker
· A medication tracking sheet designed to help organize and monitor medications. It can be used as a daily log to record the medication name, what the medication is intended for, the prescribed dosage, and the time each dose was taken, or as a general schedule to track medications, their purpose, prescribed amounts, and routine administration times.
Home Monitoring Guide
· The Home Monitoring Guide sheet provides structured templates for tracking daily observations, identifying changes in health, and monitoring the safety and function of medical equipment to support communication with healthcare providers and encourage early identification of concerns.
Budgeting Form
· Managing finances during and after cancer treatment can be challenging. The monthly budgeting form helps you organize your income, track expenses, and plan for medical and everyday costs. Using this tool can help you better understand your spending, identify areas where you may need additional support, and make informed financial decisions throughout your cancer journey.
The Role Checklist
· The Role Checklist is a self-reflection tool that helps individuals identify and prioritize the roles that are most important in their lives, such as being a student, friend, sibling, employee, volunteer, or caregiver. By considering which roles they currently have, have had in the past, or hope to have in the future, individuals can recognize what matters most to them and where they would like to focus their time and energy.
Using the Role Checklist can help prioritize daily roles by encouraging individuals to reflect on which responsibilities and activities align with their personal values and goals. This can support decision-making, goal setting, and maintaining participation in meaningful daily life, particularly during times of change or after a health-related diagnosis.[KP1]
American Cancer Society – Caregiver Resource Guide: Caring for a Loved One with Cancer
The American Cancer Society Caregiver Resource Guide is a practical guide designed to support family members and friends who are caring for someone with cancer. It provides information on managing caregiving responsibilities, communicating with the healthcare team, coping with stress, finding emotional support, and accessing helpful resources. Caregivers can use this guide to better understand their role, develop strategies for balancing caregiving with their own well-being, and locate services that can provide additional support throughout the cancer journey.
UPMC Health Library
The UPMC Health Library is a free online resource that provides reliable, evidence-based health information for patients, families, and caregivers. The library includes easy-to-understand articles on medical conditions, treatments, medications, wellness, nutrition, and disease prevention, as well as videos, interactive tools, and health calculators. It is designed to help individuals better understand their health, make informed decisions, and actively participate in managing their care.
