Health and Safety

I wish to participate with Lending Hearts.  I understand that signing this Waiver and Release is a prerequisite for participation.  I acknowledge that there are risks inherent in participating, and I agree to assume all risks related to my participation.

I agree to release and hold harmless Lending Hearts and its officers, directors, agents, employees, representatives, contractors, vendors (and their agents), agencies, sponsors, officials and volunteers (collectively, the “Released Parties”) from liability for any and all claims for damages, including, but not limited to, personal injury and/or property damage, arising from or relating to my participation, even if such liability arises from negligence or carelessness on the part of the Released Parties, or from dangerous or defective property or equipment owned, maintained, or controlled by the Released Parties.  I understand and agree that this Waiver and Release is binding upon my heirs, assigns, and legal representatives.

I understand that I am solely responsible for my health and safety.  I acknowledge that I am physically capable of participating but may be suffering from an illness that may limit my participation.  I further acknowledge officials or volunteers may authorize necessary emergency treatment for me.

Photography and Likeness:
Lending Hearts conducts a variety of events, conferences, programs, etc., and as such, it is common that we use photographs, video or “live” internet streaming to capture the events.  In an effort to get the word out about our services, we will sometimes use those photographs and/or videos for marketing purposes.  Signing this waiver indicates that you consent to the use of your likeness in our marketing efforts.

In the event that you do not consent to the use of your likeness in our marketing or promotional endeavors, please indicate as such on the RSVP form (Select NO under the Likeness option).  

Be advised that in some instances, particularly videotaped or “live” events streamed on the internet, it is not possible to exclude images of the participants based upon the nature of the event.  For that reason, there may be events which, unfortunately, we cannot guarantee that your likeness will not be captured if you chose to attend.
